Duke of Edinburgh’s Award Enrolment Form

Please read before you complete this enrolment form:

  • The enrolment fee payable on this form is a one-off fee. You can complete your DofE award anytime up to your 25th birthday, it doesn’t matter how long it takes you to finish your award.
  • Expeditions come at an adidtional cost. Participants must complete a training and qualifying expedition at all levels (Bronze, Silver & Gold). Approximate costs for expeditions run by Girlguiding South Yorkshire are as follows: Bronze £30 per person per expedition, Silver £45 per person per expedition, Gold £80 per person per expedition. These costs are a guide only and may be subject to change.
  • Girlguiding South Yorkshire organise Bronze, Silver and Gold expeditions between Easter and October half term each year. Booking forms will be sent when dates are fixed and expedition plans are confirmed. You are welcome to book an expedition with another company here if you prefer.

D of E Centre and Group

The participant must be a member of a Guide or Ranger unit or an adult volunteer in Girlguiding South Yorkshire or Girlguiding Sheffield to use this form.

Minimum age restrictions:
To enrol on the Bronze award the participant must be at least in the academic year during which they turn 14 years of age (usually year 9)
To enrol on the Silver award the participant must be at least in the academic year during which they turn 15 years of age (usually year 10)
To enrol on the Gold award the participant must be 16 years of age or older.
Maximum age restriction:
Participants have until their 25th birthday to complete any of the award levels therefore please ensure that you enrol with sufficient time prior to the participant's 25th birthday to complete the relevant award level.
Completing this form for a participant who falls outside of these age constraints may result in the loss of the enrolment fee (refunds at the discretion of Girlguiding South Yorkshire)

eDofE

When you first sign in to eDofE you will be asked to record some personal details such as your contact details, ethnicity and personal circumstances along with details of any medical needs you may have. This data is used to enable your Leaders to support you doing your DofE programme and for the DofE’s statistical and reporting purposes. You will always have a ‘prefer not to say’ option.

Declaration:

I agree to enrol as a participant on a DofE programme. I understand that I will be managing my programme using the online eDofE system. I acknowledge that this system has a set of terms and conditions that I agree to. These terms and conditions are available at www.eDofE.org

Consent to enrol from parent/guardian:

If the applicant is under 18 years of age this section must be signed by a parent/guardian. If the applicant is over 18 years of age, they can sign this section themselves to accept the below statement in addition to signing the paticipant declaration above. I agree to my daughter/ward doing a DofE programme. I note that it is my responsibility to check that any activity my daughter/ward undertakes for their DofE programme is appropriately managed and insured, unless the activity is directly managed or organised by their DofE group, centre or Licensed Organisation.

Note:

Data supplied on this form and in eDofE and information about DofE activities recorded in eDofE will be used by the DofE Charity, the Licensed Organisation and DofE centre to monitor and manage DofE participation and progress by young people and manage and support Leaders.
The DofE Charity will use personal data to communicate useful and relevant information to either help participants complete a DofE programme, Leaders/LOs to run DofE programmes more effectively or help the DofE Charity to improve the quality and breadth of its programmes.
We also send emails that contain information about the Charity, DofE negotiated privileged discounts and invites to events and other activities however if you would like to receive these emails you will need to opt in. Once you have opted-in to this you can opt out at any time by visiting www.dofe.org/preferences, or clicking the unsubscribe link that can be found at the bottom of all non-programme related email. A £1 administration fee is added by Girlguiding South Yorkshire to your enrolment fee.
